Whether it’s a little extra help around the house, clinical support from a nurse, or joining friends on a social outing, we take the time to understand what matters to you. Our friendly team works with you to tailor support that fits your needs, preferences, and lifestyle.
Home Care Services to suit you
Our team can help with the day-to-day tasks so you can enjoy the things that matter most. We’ll always do things your way, with respect for your routines and preferences.
Everyday Living
- Domestic assistance – cleaning, laundry, shopping
- Meal preparation – help preparing simple healthy meals, in home food preparation (we can batch cook and freeze meals)
- Transport – help to get to medical appointments, visit friends and relatives, attend social activities and community events. We can drop you off and pick you up in the local area (within 10 kilometres radius and 30 minutes duration).
- Gardening and home maintenance
Independence in the Home
- Personal care – showering or bathing, dressing, grooming, assistance with medication
- Social support and companionship – friendly one-on-one visits at home, going for a walk or having a coffee
- Respite care support – short-term support at home, allowing carers time to rest or attend appointments
Clinical and Health Support
- Nursing care
- Allied health – physiotherapy or podiatry

Support at Home
Support at Home is a new Australian Government program which launched on 1 November 2025. It replaced Home Care Packages and, later, will replace the Commonwealth Home Support Program to make aged care simpler, more flexible, and more personalised.
If you were receiving a Home Care Package at the time, you’ve been automatically transitioned to Support at Home.
After an aged care assessment, you’ll be assigned a support level from 1 to 8, with services funded through your individual care budget. Support available includes:

- Nursing
- Physiotherapy
- Occupational therapy
- Allied health services
Independence Supports
- Personal care (showering, grooming)
- Medication assistance
- Transport
- Social support
- Respite care
Everyday Living Supports
- Cleaning
- Gardening
- Meal preparation
- Home maintenance and repairs
Short Term Support Options
- Restorative Care Pathway: Up to 12 weeks of allied health support to help you regain strength and independence.
- End of Life Pathway: Compassionate care to support people with a life expectancy of three months or less to remain at home with comfort and dignity.
- Assistive Technology and Home Modifications: Access equipment and home modifications — such as walkers, mobility scooters, or safety rails — to help you stay safe and independent.
Commonwealth Home Support Program
The Commonwealth Home Support Program is for older people who are mostly independent but need a little help with daily tasks. This is an entry level program for subsidised support to help you stay independent, safe and healthy at home.
Typical services include:
- Cleaning, shopping, and food preparation
- Personal safety alarms and safety modifications
- Social activities and transport
- Allied health support
Commonwealth Home Support is often the first step in getting aged care support but is not as comprehensive as Support at Home. You can check your eligibility and apply through My Aged Care.
Private Services
If you’d prefer to start services straight away, or don’t wish to access government-funded programs, you can also pay privately for home care through Jubilee Community Care.
Our private care services include domestic assistance, personal care, nursing, allied health, respite, and palliative care — all delivered by our experienced care workers.
There are no waiting lists and no restrictions on how often you receive services.

We support older people living in Brisbane’s western and northern suburbs. Services may vary depending on your location and funding type, so please contact us to check what’s available in your area.
Can I choose the day and time my care worker visits?
Do I need a Support at Home package to access these services?
No — a Support at Home package is not required to get support from us. While many of our clients do use government-funded packages, we also offer privately funded services.
If a Support at Home package may be the best option for you, our team can provide tailored advice and help guide you through the application process.
Can I get just a little bit of help, or do I need to sign up for a package?
You can receive as much or as little support as you need. We offer flexible services that can start small and change over time.
How much does home care cost? How do I pay for home care?
The cost of home care assistance depends on the services you receive and the funding you’re eligible for. Costs may be covered fully or partly by government funding, with some out-of-pocket contributions in certain cases.
How do I access home care funding?
Accessing funding usually starts with an assessment through My Aged Care. Our team can guide you through the process and help you understand what support you may be eligible for.
What training do your staff receive?
Our home care staff receive comprehensive training and regular professional development, including mandatory annual training and specialised education relevant to aged care and chronic conditions.
Can I change the type of help I get as my needs change?
