Care Management
Under Support at Home, 10% of your ongoing quarterly budget is allocated to Care Management. Your Jubilee Care Partner works with you to develop and review your care plan, coordinate your services, monitor your budget and respond as your needs change. You are not billed by the hour for Care Management. The table below shows the quarterly budget and corresponding 10% Care Management allocation for each Support at Home classification.

Restorative Care Pathway
The Restorative Care Pathway provides around $6,179.40 for up to 16 weeks of short-term, intensive support to help you maintain or regain your independence. If additional services are needed, a further funding unit may be approved, providing up to $12,358.80 for the episode.
Your Jubilee Restorative Care Partner will work with you to set and review goals, coordinate services and monitor your progress.
Care Management is charged directly to your Restorative Care Pathway funding rather than through the usual 10% allocation. There is no fixed cap, and the amount will be agreed with you based on your needs and available funding.
Assistive Technology & Home Modifications (AT-HM) Pathway
If you are approved for Assistive Technology or Home Modifications your Care Partner can help coordinate the equipment, modifications and associated services you need.

Understanding your contributions
Depending on your circumstances, you may contribute towards the cost of some Support at Home services. Clinical Supports have no participant contribution, while contributions may apply to Independence and Everyday Living services. Your Care Partner can explain what this means for you.
Private Services
You don’t need government funding to receive support from Jubilee. Private services are available if you’re waiting for funding, would like additional support, or simply prefer to pay privately. Our private service prices are generally the same as our Support at Home prices.
More than just the service
With ongoing private services, you’ll also have a dedicated Care Partner to help coordinate your support as your needs change.
Cancellations
We understand that plans change. Please give us as much notice as possible if you need to cancel or change service. Cancellation fees may apply where less than 48 hours’ notice is given, in line with your Service Agreement and Support at Home requirements.
