Care Partners
Your first point of contact at Jubilee. Care partners:
- Schedule your services and match you with the right support staff.
- Keep you informed about any changes.
- Answer your questions and guide you through your care options.
They’re skilled communicators who make sure your care is well organised, flexible and stress-free.
Care Workers
Our largest group of staff, and the people you’ll see most often at home. Care workers help with:
- Daily living tasks – meal preparation, cleaning, shopping.
- Personal care – showering, dressing, grooming.
- Companionship and outings – joining social activities or enjoying time in the community.
Our care workers are chosen for their kindness and dedication and trained to provide high quality, respectful care. We carefully consider your needs and preferences when arranging your support, so you can enjoy consistent, compassionate care.
Registered Nurses
Some clients need extra health support. We provide registered nurses who help with:
- Health assessments
- Medication management
- Wound care and other clinical services
They work closely with you, your family, and your GP to keep your health on track.
Allied Health Professionals
Depending on your needs, you may also see visiting specialists such as:
- Physiotherapists – to support mobility and strength.
- Occupational therapists – to recommend equipment and home modifications.
These are highly qualified professionals focused on keeping you safe and independent at home.
Working Together for You
Every client’s team is different — some people mainly work with their care worker, while others also receive nursing or allied health services. What’s consistent is that all our staff are connected through your care coordinator, who ensures your services meet your needs and preferences.

Diane Evans
Executive Director
Diane Evans is Executive Director at Jubilee Community Care, bringing over 20 years’ experience in people leadership, organisational development and business consulting across corporate, not-for-profit and small business sectors.
She cares deeply about creating supportive, values-driven teams and environments where both clients and staff can thrive. Diane is passionate about enriching the lives of older people and is committed to ensuring every person is supported with genuine care, respect and dignity.
Mardy Abeydeera
Financial Controller
With over 25 years of experience in financial management, Mardy is an accomplished Financial Controller with extensive knowledge built across both large and small organisations.
She brings expertise in not-for-profit financial operations, including accounting, reporting, grant management, budgeting, compliance and strategic planning, and is committed to supporting sustainable, mission-driven outcomes.
Known for her integrity and focus on transparency, Mardy plays a key role in ensuring strong financial stewardship at Jubilee. Outside of work, she is passionate about giving back to the community, including supporting animal rescue initiatives and helping children from disadvantaged backgrounds access education and opportunities.
Sharon Forbes
Client Services Manager
Sharon is a Client Services Manager and Registered Nurse with extensive experience in community aged care, client services, and business development. She is known for her warm, approachable nature and her genuine commitment to supporting clients to live with dignity and independence.
Combining clinical expertise with strong relationship-building skills, Sharon works closely with clients, families, and teams to deliver personalised, high-quality care. She is passionate about creating positive outcomes while contributing to service growth and innovation within the aged care sector.
Sharon values health, wellbeing, and time with family, and brings compassion, resilience, and a people-focused approach to everything she does.
Jean Yan
Care Manager
With over 20 years of nursing experience, Jean brings a wealth of knowledge, leadership and genuine care to her role as Care Manager at Jubilee Community Care.
Since joining Jubilee in 2023, Jean has supported clients and their families with a practical, person-centred approach. Her background spans both clinical care and leadership, including roles as a Facility Manager and Clinical Manager in aged care, giving her a strong foundation in clinical governance, case management and compliance.
Jean is passionate about supporting clients in a practical and individualised way, helping them maintain their independence and achieve their personal goals. She values building genuine relationships with clients, families and staff, and is committed to delivering care that is respectful, flexible and tailored to each person’s needs. Jean takes pride in making a positive difference, no matter how big or small.
